competition. Important rule changes and reminders will also be listed on
the Updates
page.
We
offer the entry form in two Acrobat PDF formats:
A Standard
Format which you may simply download, print out, fill in by hand and
fax or mail to show organizers.

Or, you can
open (or download and open) this fillable entry form, fill in with
your computer and print it out. (Note: you will NOT be able to save
the filled-in form)

To download
and print these pages, you will need Acrobat

Helpful
Hints
- Riders, trainers,
owners, coaches and show committees have learned that working together
is fruitful for all parties involved. There are, however, a few points
to keep in mind when entry forms are mailed to the show secretary: Mail
a copy of your current USEF and USDF Cards with your entry (Rider,
Horse, Trainer, Owner, Coach).
- Fill out your
entry form with a dark pen. Provide clear information when asked for.
Look at the policies of the show you enter; shows have different
policies in many ways.
- Make ample (two
sided) copies from the entry forms provided with the "Blue Book". These
entry forms are up to date for USEF and FEI competitions. Other entry
forms may not be accepted.
- Give sire and dam
information to support the recognition of the breeders of your fine
horses.
- Please be a sport
- each entry form must have ALL signatures (USEF
Requirement).
- Inform yourself
about refund policies. With your signature, you are accepting all
policies of the show you enter.
- Be prepared to
furnish the show office with all missing credentials and dues. Many
shows will not issue your bridle numbers until you have done so. Your
horse must wear numbers during the entire time it is on the grounds and
out of its stall (USEF Rule).
- If you have any
questions during the show, go to the appropriate person to help you:
show office to answer all questions regarding the show; ring stewards to
check your tack/bits and keep you informed about warm-up and competition
rings; technical delegate to interpret the rules; and the show committee
to follow up on rule violations.
